Step 1: Gather Required Documents
To complete the registration process, you must provide documentation proving your child’s date of birth as well as your identity and/or parental/legal guardianship status, and your family’s home address. You are also required to provide a completed Health Assessment Record for your child to be able to attend school.

Required Documents:
- Birth certificate or official documentation proving the child's age and parent(s) name(s).
- Photo ID of child’s parent or legal guardian. Please note, proof of legal guardianship is required for students who do not live with their parents.
- Proof of address (2 forms). Please refer to the proof of address guidelines for detailed information:
(If you live outside of Stamford and your child is attending Rogers International School (K-8), Strawberry Hill School (K-8) or AITE (9-12), please provide proof of address for the town in which you live.)
- If you rent your home and are listed on the lease, you must provide:
- A current lease
- 1 current utility bill (gas, electric, water, internet, cable, or phone).
- If you rent your home but are NOT listed on the lease, you must provide:
- Notarized residency form
- 2 current utility bills (gas, electric, water, internet, cable, or phone).
- If you own your home, you must provide
- The deed, a current tax bill, or a current mortgage statement
- One current utility bill (gas, electric, water, internet, cable, or phone)
- Health Information. Proof of a current physical exam and up-to-date immunizations are required before your child can attend school. Please ask your pediatrician’s office to complete the State of Connecticut Department of Education Health Assessment Record (Form HAR-3). The parent or legal guardian should complete, sign, and date page 1; the physician’s office will complete pages 2 and 3.
If your child has a documented medical condition that will require medication to be administered during the school day, ask the prescribing physician to complete the Authorization for Administration of Medication form.
