Steps to Completing Student Registration
Step 1
To register a student for school you must provide:
- Birth certificate or official documentation proving child's age and name of parents
- Photo ID of parent or guardian
- Registration is by parent or guardian only
- Proof of guardianship is required for students not living with parents

2 forms of Proof of Address: Please see our Proof of Address guidelines for more information. *If you have been accepted into AITE, Rogers or Strawberry Hill and reside out of Stamford please provide proof of address in the town you live.
- If you rent:
A current lease AND a current utility bill (phone, gas, electric, cable, or water)
If you are not listed on a lease you must have a notarized residency form completed and provide two current utility bills (phone, gas, electric, cable or water)
- Notarized Residency Form A (English)

- Fòm rezidans notarye A (Ayisyen)
- Нотаріально завірена форма проживання A (українською мовою)
- If you own:
A deed or current tax bill or bank mortgage showing ownership of a residence in Stamford AND a current utility bill (phone, gas, electric, cable or water)

Health Requirements: Current physical exam and immunization records are required to attend school (although you can still register your child in the school).
Please ask your doctor's office to use the Health Assessment Record (HAR-3). Parents need to fill out the first page, sign it, and date it. Page 2 and 3 should be filled out by the Dr.

Other Registration Forms:
After completion of the registration application, documents can be submitted in different ways:
- You can upload the documents you already have directly into the application before submission
- Documents may be dropped off at the Government Center by calling in advance at (203) 977-5312
- You can make an appointment with your child's school
- You can Email your documents to: OFCE@stamfordps.org
